Our program is structured into two distinct phases.
Phase 1 focuses on preparing young adults aged 18-21 for independence through the Host Family Model. This phase emphasizes personalized education and career plans, communal activities, and mental well-being to foster a sense of belonging and self-sufficiency.

Phase 1: Host Family Model
The SOS Project is excited to announce the grand launching of our Transitional Housing initiative in Summer 2025, specifically designed for former foster youth ages 18-21. This program aims to provide essential support and resources for these young adults as they transition into independent living. By creating a safe and nurturing environment, we will help them gain the skills needed to thrive in their personal and professional lives.
A key focus of our initiative is to connect former foster youth with vital community resources. Through partnerships with local organizations, we will offer workshops, mentorship programs, and access to job training, education, and health services. Our goal is to empower these young individuals with the tools they need to build strong, sustainable futures and foster connections with their families and communities, ensuring they never feel alone in their journey.

EXTENDED FAMILY CARE PROGRAM
PROGRAM:
Weekly meetings will be conducted to facilitate ongoing dialogue among participants, administrators, and support staff. These meetings will serve several critical functions, including monitoring progress by reviewing each participant's advancement towards their individualized education plans. This collaborative environment ensures that everyone stays informed and engaged, fostering a supportive atmosphere where participants can share their experiences, challenges, and successes. Additionally, these sessions provide an opportunity for administrators to offer guidance, address concerns, and make necessary adjustments to support each individual’s journey towards stability and success in transitional housing.
